The following links provide example problems and related information that could be helpful to those who choose to participate in the Requalification Examination to be given on November 28, 2012 and November 29, 2012. Please be advised that if you chose to take the Superpave Mix Design Technologist Exam, both of the following review links should be considered as study material. Along with your notification letter that was sent to you by mail, you should have received a packet of review material containing relevant AASHTO Standards. Asphalt Plant Approval

For initial asphalt mixing plant approval, the contractor must submit the “Contractor's Master Certification of Asphalt Mixing Plants and related Equipment” to the Department , this form should be completed during an actual plant inspection by contractor personnel directly responsible for the plant operations. 39 kb, 11 pages, updated 04/22/2009.
After initial approval of an asphalt mixing plant, annually thereafter (prior to production of asphalt mixture for Department projects) submit to the DME the following form for plant approval: “Annual Certification for Previously Approved Asphalt Mixing Plants and Related Equipment”. 17 kb, 4 pages, updated 04/08/2009.
At the beginning of the construction season, to obtain approval for each asphalt mixing plant, submit the following form to the DME: “TC 64-418, Contractor's Quality Control Plan/Checklist”.
