are the steps to take in becoming an approved Non-Emergency Medical (Medicaid)
Transportation Private Auto Provider (NEMT).
To be an approved Private Auto Provider, the applicant must
be a private individual (non-business) using a private vehicle to transport a
Medicaid eligible recipient to a Medicaid covered service at a Medicaid
approved medical provider. An approved
Private Auto Provider will be reimbursed a set mileage rate for eligible
transports. Recipients must meet all transportation eligibility requirements
and be approved by the Regional Broker prior to a Private Auto providing
transport. A recipient who is
transported by a Private Auto Provider cannot be a family member of a Private
Auto Provider and cannot live in the same household as a Private Auto Provider.
To apply:
- First, the applicant must contact the Regional Broker for the county in which the Private Auto individual intends to operate and/or where the recipient the applicant intends to transport resides. For a listing of Brokers and the counties they serve, please select the following link: Human Services Transportation Brokers. The applicant will request a Private Auto application packet from the Broker.
The applicant will complete the application packet and submit all required information to the Regional Broker. Please note: all documents required must have effective dates as of the date of the application submitted.
The Broker will review the application, ensure a background check is conducted, sign the application, and then forward the completed application and supporting documents to the Kentucky Transportation Cabinet.
The Kentucky Transportation Cabinet will review the application, verify driver license and vehicle registration, sign the application, and then forward the application and supporting documents to Medicaid Provider Enrollment.
Once enrolled as an approved Transportation Private Auto Provider, the applicant will receive an approval letter from Medicaid Provider Enrollment noting the approved Private Auto Provider's assigned Medicaid Provider ID Number.
The approved Private Auto Provider must forward a copy of the approval letter to the Regional Broker. The Regional Broker will train the approved Private Auto Provider on program eligibility and billing/invoicing procedures.
Things to remember: Only individuals approved as a Private Auto Provider may transport eligible recipients under the Private Auto Provider Program. Only approved vehicles may be used to transport eligible recipients under the Private Auto Provider Program.
