Please identify the statewide crash database and provide a description of the statewide database, specifying how the data is consolidated. If the State has existing documentation (data dictionary, schema, reports, summaries, etc.), please submit the relevant document or an excerpt thereof. If the State does not have existing documentation, a brief narrative may be submitted instead.
