Motor Carrier Information - Includes Vehicle Inventory for KYU
Motor Carrier Information
To be used for verifying contact and carrier information. Displays mailing address, physical address, USDOT#, phone and fax numbers, e-mail provider (not address), and a button to display vehicle inventory for KYU (Kentucky Weight Distance Tax) holders. Displays the last 6 digits of the VIN only.
Vehicle Additions or Deletions of Vehicles in Kentucky Subject to Kentucky 
Web based. Used to add or delete a vehicle that operates under a KYU number. Must be signed up for KYU electronic tax filing to view this inventory on-line feature that displays the full 17 digit VIN.  Customers that have not signed up for ETF and want to view their vehicle inventory on-line for KYU, please use the Motor Carrier Information link above. Last 6 digits of VIN only will be displayed.

FORM ONLY TO BE USED WHEN THE ABOVE LINK IS UNAVAILABLE. The vehicles to be added or deleted are those vehicles subject to the Kentucky Weight Distance (KYU) Tax. Only vehicles having a "Combined Licensed Weight" in excess of 59,999 pounds are to be included in this list.

Authorization Agreement for Preauthorized Payments (OWOD)
Must sign up for this service. To order ow/od permits on-line please complete the Authorization Agreement for Preauthorizes Payments form. Available for ACH debit only. Please read all instructions and include a voided check when mailing or faxing to the office.
Permit Service/Wire Service
Web applicants for permits must have an agreement on file with the Division of Motor Carriers authorizing the ACH debit of your bank account for any permits issued to you or your company. No credit cards will be accepted for payment of Web based permits.
Trucking Companies
Web applicants for permits must have an agreement on file with the Division of Motor Carriers authorizing the ACH debit of your bank account for any permits issued to you or your company. No credit cards will be accepted for payment of Web based permits.

Weight Distance License (KYU) Application
Web applicants for weight distance licenses must have an agreement on file with the Division of Motor Carriers authorizing the use of this system. Please allow 24-48 HRS for the Motor Carrier's Insurance and UCR to load into our system before you apply for the KYU.
