Post-construction Review

Post-construction review of the subject project was held on June 19, 2001 in the District 2 conference room with the following personnel in attendance:

The attendees reviewed the plans and the change orders incurred during the construction of the subject project. A total of five (5) change orders had been issued for this project. The meeting was concluded with an on-site investigation of the project area. The project involved pavement rehabilitation of a section of Western Kentucky Parkway from IC Railroad Bridge at MP 55.52 to MP 59.85 for a total of 4.33 miles. The meeting dealt with issues pertaining to Guardrail, Edge Drain, and Surface. The following is an account of the incurred change orders.

CHANGE ORDER NO. 1

This change order was approved in part to upgrade the planned erosion control scheme by using Erosion Control Blanket in the entire area of the median. This change was deemed necessary in order to overcome the recurring difficulty in achieving seed germination. This change yielded the desired vegetative cover. This change order also added Temporary Bridge End Connectors Type A and C. These items which were not included in the plans, were needed to protect the bridge ends that would otherwise be exposed when traffic would be crossed over in the opposing direction. Other appropriate ancillary items such as temporary guardrail and end treatments were also added as required. All end treatments became the property of the Department after removal and were delivered to Muhlenburg Maintenance Facility.

An extension of the contract working days was not noted in this change order.

Increase $38,611.70

CHANGE ORDER NO. 2

This change order was issued to perform the following changes and/or modifications;

  1. Replace Guardrail End Treatment Type 2A with Type 7 to meet the current geometric standards. These were installed on KY 604 and KY 2697.
  2. Replace the "Motor Transfer Vehicle Paver" with "Cedar Rapid Remix Paver", since the asphalt plant was not producing enough material to keep up with the MTV Paver. This change did not result in additional expenses.
  3. Add the required items, namely delineators for guardrail and tubular markers, to safely channelize traffic on US 431 and the transitions on the Western Kentucky Parkway. The said items were called for in the standard drawing but left out from the pay items.
  4. "Remove and Replace Slope Wall" at the KY 2697 Bridge due to a cavity encountered below the wall.
  5. Add "Structure Granular Backfill" to properly backfill the US 431, KY 2697, and KY 604 bridge ends. This item which was required by the pertinent Standard Drawing had not been set up on the plans.
  6. Add a junction box. This item had been overlooked and was needed to connect the cross-drain and the median drain at Sta. 4439+00.

An extension to the working days was not addressed in this change order.

Increase $11,114.50

CHANGE ORDER NO. 3

This change order allowed the use of Cedar Rapids Remix paver in lieu of the MTV on the remaining section of the project from MP 56.03 to MP 59.85. This change was recommended by the Resident Engineer and resulted in approximate savings of $800,000.00. This change order also added additional quantities of Pavement Removal from the shoulder areas. This work was originally set up. However, quantities were not included in the plan summary. This change order also allowed additional quantities of PCC Pavement Repair to repair the concrete, which had deteriorated in the areas where traffic had been shifted throughout the winter months.

Eight (8) additional working days for Change Order 1 and Change Order 2 were allowed by this change order.

Increase $216,315.00

CHANGE ORDER NO. 4

This change order was issued to perform the following changes and/or modifications;

  1. Allow use of Aggregate Size No. 1 instead of No.3 in ditches and cut sections for erosion control. The contractor had Aggregate Size No. 1 available on site. Hence, the decision was made to replace Aggregate Size No. 3 with No. 1. This change did not result in any additional expense.
  2. Remove the existing sign support beams in order to install the new signs. The existing sign support beams were left off the previous Western Kentucky Parkway signing project and would have to be removed before the new Sign Bridge Attachment Brackets could be installed.
  3. Add Safety Box Inlets to replace the existing ells and straight headwalls. These items had not been addressed on the plans.

Fifteen (15) additional working days were allowed by this change order. Nine (9) days were for not allowing the contractor to work because of Memorial Day and Independence Day holidays. Five (5) days were allowed per the TEBM for Construction to avoid changing the plant over during the surfacing operations. And one (1) day was allowed for the actual work done.

Increase $39,690.00

CHANGE ORDER NO. 5

This change order was issued to perform the following changes and/or modifications:

  1. Delete unused quantities of Flowable Fill and Asphalt Wedge Curb.
  2. Modify the restricted width of mine haul road bridges with "Bar Splice Phase Construction" to widen the lanes from 11ft. to 15 ft.
  3. Add Fence-Woven Wire and repair the damage on the existing fence at the bridges. This item had not been set up on the plans.
  4. Repair an existing crash cushion at US 431 and a cavity at Sta. 4314+00. The cause for the sudden development of the cavity had not been determined at the time of the post-construction review, but is being investigated by the Geotechnical Branch.

No additional working days were allowed by this change order.

Change orders resulted in an increase of 2% in the total project cost.
