Post-Construction Review
Dear Ms. Wagner-Justice:
A post-construction review was held on the subject project on February 25, 2000 in the District 12 Conference Room. The following personnel were in attendance:

The attendees reviewed the project plans and construction Change Orders. There were 4 (four) Change Orders on the project. An on site investigation followed the meeting. This post-construction review meeting dealt with grade, drain and bituminous surface of Hager Hill Connector from 250 feet east of KY 825 to US 23.
The following Change Orders were discussed:
CHANGE ORDER NO. 1
An access ramp located between KY 1428 and KY 321 was to be closed during construction of a culvert, so it was decided that it would be feasible to reconstruct this ramp as part of the contract for this project. This work involved breaking and seating the existing concrete pavement, placing two courses of bituminous base and one course of bituminous surface. Also, the existing shoulders were reconstructed using the same pavement design. DGA and chip-seal shoulders were installed and the existing guardrail was replaced.
From approximate station 124+00 to station 142+50 it was determined that the existing original ground was very soft and unstable. It was decided to raise the proposed grade in order to bridge over this existing area in lieu of undercutting this area. This was accomplished by applying type III filter fabric with a rock fill at the request of the Geotechnical Branch.
Also, upon field inspection, it was determined that the area where the retaining wall was to be built left station 120+00 to 123+00 was unstable. In order to stabilize this area, filter fabric type III with a No. 2 stone backfill was used.
Sedimentation basins were placed at locations specified by the Best Management Plan (BMP) for this project. Also, an existing water well at approximate station Rt. 141+00 was plugged, since the dwelling had been removed.
An increase of seventeen (17) Working Days was allowed for this Change Order.

CHANGE ORDER NO. 2
A drainage structure was placed at Station 125+84. This structure was needed to control unanticipated run-off in this area. It was determined in the field that sufficient clearance was not available to install a Type 4 end treatment on the access ramp between KY 1428 and KY 321 as designated in the plans and a Type 1 end treatment was installed in lieu of the Type 4.
After construction of a retaining wall Lt. Station 118+38 to Station 123+00, it was decided to install a 6-foot chain link fence to the top of the wall in order to protect pedestrians. The wall had to be drilled in order to anchor the fence posts. As to date, there have been no cracks in the wall due to drilling.
Any wall four feet or higher in any area should be designed with a six-foot high minimum chain link fence.
Working Days were unaffected by this Change Order.

CHANGE ORDER NO. 3
The contract plans included headwalls throughout the project. However, no concrete or steel quantities were included in the bid items for the construction of these small drainage structures. Also, the contractor performed work in order to restore the septic systems for parcel #13. The construction of the adjacent retaining wall destroyed septic systems at both residences on this parcel. An agreement with the property owner at this location indicated that if any damage at all was done to the septic system then a new system would be installed.
The pavement design on this project was changed to 4" of DGA, 9 ¾" Bituminous Base and 1 ½" Bituminous Surface. The initial pavement design utilized Bit. Coated Drainage Blanket and 10" of DGA. This was done to accommodate the traffic under phase construction. When maintenance of traffic plans indicate placing traffic on the driving lanes prior to placing bituminous mixtures, it is not recommended to use bituminous treated drainage blanket.
Contract Working Days were unaffected by this Change Order.

CHANGE ORDER NO. 4
Rock Borrow quantities were established in Change Order No. 1. This Change Order was necessary to increase the quantity for final payment. This area was cross-sectioned to establish the actual quantity of "rock borrow" used.
Contract Working Days were unaffected by this Change Order.
Increase $25,960.00
Additional comments were as follows:
The Railroad should have been involved with this project during pre-construction. The railroad was never notified of this project and work was being performed on railroad right-of-way.
Material that is excavated from ditches, etc. should not be used in the roadway fill. Material that is excavated should be better classified, as it is not always suitable for use in the main line.
There was a 12’ high gravity retaining wall constructed on this project. Right-of-way is only 2 feet behind this wall. If there is ever a problem with this wall, there is no means to get in this area to do any work. There was only a temporary easement established. It was felt that a permanent easement should have been established to allow the Department a means for any corrective or maintenance work to this wall.
