Post-Construction Review

Post-construction reviews were held on the subject projects on February 24, 1998 in the District 10 conference room. The following personnel were in attendance:

The attendees reviewed the project plans and construction change orders. An on site investigation of both projects followed the meeting.

The following change orders were discussed:

BREATHITT -WOLFE COUNTIES KY 205

CHANGE ORDER NO. 1

This change order was necessary because culvert footings at three different locations were redesigned using floating footer pads. This was necessary due to rock elevations being found at different elevations other than what was designated on the plans. Also, sloped inlet and outlets were added on entrance pipes to prevent erosion and preserve shoulder stability.

Pavement thicknesses were increased due to updated traffic projections as per the Central Office, Division of Design.

Contract time was unaffected by this change order.

Increase $616,730.62

CHANGE ORDER NO. 2

This change order was necessary to permit the use of performance grade asphalt binders conforming to section 806.06 of the supplemental specifications.

Contract time and funds were unaffected by this change order.

CHANGE ORDER NO. 3

This change order was necessary because of the occurrence of several pop slides on the project. The material from these slides had to be removed from the mainline roadway which caused an overrun of Roadway Excavation. This material was hauled an average distance of 8100 feet due to the unavailability of a waste area, which resulted in an overhaul situation.

Contract time was unaffected by this change order.

Increase $60,067.95

MENIFEE COUNTY KY 1274

CHANGE ORDER NO. 1

This change order was required to compensate the contractor for additional expenses incurred to accelerate the project. The project was accelerated to allow the opening of the road to traffic going to the Cave Run Lake recreational area for the Fourth of July Holiday.

Contract time was unaffected by this change order.

Increase $25,542.89

CHANGE ORDER NO. 2

This change order was required to increase quantities of guardrail and bituminous paving items. These additional quantities were needed to pave and guardrail a Forest Service Boat Ramp and Parking Lot as per the Division of Construction and the State Highway Engineer’s Office. Also

 

included in this change order were the actual labor costs and incentive payments necessary to accelerate the project as stated in Change Order Number 1.

Contract time was unaffected by this change order.

Increase $69,911.51

CHANGE ORDER NO. 3

This change order was necessary to increase plan quantities of seeding and protection. There was not enough seeding quantities set up in the original contract to address seeding the ditch areas. These additional seed quantities helped expedite erosion control. Also, Silt Fence was added to the contract in order to control the amount of silt that would enter the Cave Run Lake. The contractor was also compensated for correcting a rock fall that occurred on the project.

Contract time was unaffected by this change order.

Increase $23,911.81

CHANGE ORDER NO. 4

This change order was necessary to establish additional quantities for Fertilizer 20-10-10. The contractor was advised on the final inspection that the cuts on the project needed to be top dressed.

Contract time was unaffected by this change order.

Increase $1,200.00

Additional comments from these post-construction reviews were as follows:

BREATHITT-WOLFE COUNTIES

The area between the new road and the existing facility could have been used for a waste area. There was a property in this area that wasn’t bought during R/W negotiations, but the Department ended up condemning the property.

The solid rock line was lower than what was shown on the plans for all 3 culverts on this project. It was suggested that the contractor and Resident Engineer in the future check the surface elevations on drill holes in order to confirm plan elevations.

The culvert at approximately station 258+00 was constructed part-width. This was very complicated to build and guardrail had to be installed in order to maintain safety in this area.

The plans called for traffic to be maintained on DGA Base. Due to heavy truck traffic the pavement design was altered to allow the use of Bituminous Base for maintenance of traffic.

Some bench marks were off by as much as one foot on this project. It was determined that levels should be checked more closely using a level in lieu of using trigonometric leveling.

 

 

 

Utility lines on this project were a problem and had to be relocated a second time. There was not adequate clearance during construction for the contractor to fit equipment.

District personnel would like to see a note on the plans stating that cuts should be seeded as they are brought down.

Also district personnel would like to see a note on the plans concerning salvaging and replacing signing. The existing signs were taken down by the contractor and stored at a waste area. Most of the signs ended up destroyed or lost.

There were concerns about the beginning and ending project tie-ins. There were grade differences in these two areas which resulted in forming a pocket of water. This area needed to be designed for pipe but no consideration was given to these grade differences. It was felt that the project may have been tied down at an undesirable point.

 

MENIFEE COUNTY

There was no detour possible on this project; therefore, the road had to be closed during construction. The mail carrier had to travel thirty two additional miles in order to deliver mail on this route. There was a tremendous amount of local opposition to the road being closed during construction, and this was part of the reason why the contractor was given the incentive to accelerate the project.

The area surrounding the lake was used as a waste site. Material placed in this area could not spill into the lake. The slopes were steepened to 1 ½:1 and were dressed with a track hoe because a dozer couldn’t be used in these areas. It was suggested that barrier wall could have been used for permanent rock fall protection.

There was no erosion control set up on this project.
