Post-Construction Review
Dear Mr. Coffey:
A post-construction review for the subject project was held on October 21, 1999 in the District 8 Conference Room. The following personnel were in attendance:

The attendees reviewed the project plans and construction Change Orders. There were seven (7) Change Orders for the project. An on site investigation followed the meeting. This post construction review meeting dealt with Grade, Drain and Bituminous Surface on the Burkesville-Columbia (KY 61) from Jones Chapel Road and extending northwesterly to 150 feet south of the Adair County line.
The following Change Orders were discussed:
CHANGE ORDER NO. 1
This Change Order was necessary to add items in order to provide adequate erosion control throughout the project. This project was let prior to the requirement of erosion control plans. Additional silt checks, Silt Fence, and one Silt Trap Type A were required to provide adequate erosion control.
An increase of 1 Working Day was allowed for this Change Order.
Increase $8,800.00
CHANGE ORDER NO. 2
This Change Order was necessary to revise the mainline and shoulder pavement design throughout the project. The original plans called for one layer of base and surface and earth shoulders. However, the Division of Design Pavement Branch made a pavement design revision utilizing two courses of base and paved shoulders after being provided with updated traffic projection information which indicated this route would have significant truck traffic. Also, two adjoining projects were changed to the same pavement design.
An increase of 39 Working Days was allowed for this Change Order.

CHANGE ORDER NO. 3
This Change Order was necessary to install 54-inch pipe in lieu of 42-inch culvert pipe as shown on the plans at Station 222+60. Current fill height charts required the larger pipe at this location due to a 90-foot embankment. Also this Change Order was necessary to allow the use of Class IV Channel Lining for the riffle structures in lieu of "Riffle Structure-Dumped Stone." Due to the ample supply of rock on the project the project engineer allowed the contractor to use this material. The structures were constructed in accordance with the plans and specs and the material met the same specs as the original bid item.
An increase of 1 Working Day was allowed for this Change Order.

CHANGE ORDER NO. 4
This Change Order was necessary to install Type 4A end treatments in lieu of Type 4 End Treatments in accordance with the current design standard for guardrail systems. Additionally, while excavating for the box culvert at station 185+80, unsuitable rock was encountered at the plan footing grade and an additional quantity of concrete was needed in order to increase the footing depth down to sound rock as required by the plans. Additional excavation quantities were also necessary at this location.
An increase of 1 Working Day was allowed for this Change Order.

CHANGE ORDER NO. 5
This Change Order was necessary to excavate additional material from rock cuts Lt. Station 212+00 to 217+50 and Rt. Station 217+50 to Station 218+00. Slope changes and additional excavation was required due to fractures and mud seams remaining in the rock walls after the cuts were originally presplit and excavated. Also, a well that was encountered during construction at Station 152+75 had to be capped and filled in accordance with Section 711 of the Standard Specifications.
An increase of 3 Working Days was allowed for this Change Order.

CHANGE ORDER NO. 6
This Change Order was necessary to construct a drop box inlet in lieu of a sloped & flared headwall at the inlet of the 24-inch culvert pipe Lt. of Station 272+40. This change allowed the new ditch grade to be raised and the fill slope to be flattened in this area which eliminated the need for guardrail Lt. Station 269+25 to Station 274+00. A ditch was channel lined Rt. Station 120+00 to Station 125+50. Also, Crushed Aggregate No. 8 Stone was added in order to backfill culvert pipes at Stations 111+25, 114+40, and 164+00. The pipes at these locations were installed during part-width construction of the roadway in order to facilitate traffic. These pipes were located in rock fills and had shallow cover heights.
An increase of 1 Working Day was allowed for this Change Order.

CHANGE ORDER NO. 7
This Change Order was necessary due to encountering unsuitable material at various locations on the project. The contractor, at the discretion of the Engineer undercut these areas and an additional quantity of roadway excavation was needed to complete this work. Also, a slope revision was made on the project Lt. Station 133+00 to Station 135+00, which required additional excavation. A guardrail end treatment was changed Lt. Station 261+25 from a Type 2A to a Type 1. This change was necessary to install an end treatment that was best suited to the finished roadway template.
An increase of 2 Working Days was allowed for this Change Order.
