Post-Construction Reviews

Dear Mr. Mosley:

The post-construction reviews for the subject projects were held on June 22, 1999 in the District One conference room. The following personnel were in attendance:

The attendees reviewed the project plans and construction Change Orders. There were 11 (eleven) Change Orders for the Husbands Road project and 1 (one) Change Order on the Bridges over KY 70. On site investigations followed the meeting. These post-construction review meetings dealt with the grade, drain and bituminous surface of Husband Road (KY 1954) from SEUL of Paducah extending northerly to the intersection of US 60 and US 62 in McCracken County and bridge replacements on bridges over Dunn Creek, Hickory Creek, and Sugar Creek on KY 70 in Livingston County.

The following Change Orders were discussed:

CHANGE ORDER NO. 1 (MCCRACKEN COUNTY)

This Change Order was necessary to substitute Crushed Stone Base in lieu of DGA Base. Since the contractor was able to obtain Crushed Stone Base at the same cost of DGA it was decided to use Crushed Stone Base. It was felt that Crushed Stone Base would also better facilitate drainage of the subgrade.

Contract time was unaffected by this Change Order and there was no net increase or decrease to the original contract amount.

CHANGE ORDER NO. 2 (MCCRACKEN COUNTY)

This Change Order was necessary to provide additional quantities of geotextile fabric and granular embankment throughout the project. Extremely swampy conditions were encountered when excavation on the project began. It was not possible to construct the 8" cement modified roadbed as was required in the plans. It was decided to raise the grade and construct a 16" granular embankment with geotextile fabric. This alternative facilitated drainage and provided a working platform on which to construct the new pavement system.

An increase of 5 Working Days was allowed for this Change Order.

CHANGE ORDER NO. 3 (MCCRACKEN COUNTY)

This Change Order was necessary to establish bid items for electrical conduit and junction boxes for the barrier walls of the bridge over the P & L Railroad. The contract plans required these items but they were inadvertently omitted from the contract bid items. Also a construction revision was made in order to install lighting on the bridge and contract bid items were set up to facilitate this revision.

An increase of 2 Working Days was allowed for this Change Order.

Increase $66,955.00

CHANGE ORDER NO. 4 (MCCRACKEN COUNTY)

This Change Order was necessary to install traffic signals at the intersection of New Husband’s Road and Frontage Road No. 4. Because Frontage Road No. 4 now handles all traffic from former Oaks Road and Old Benton Road, the Division of Traffic requested that traffic signals be installed at the intersection of New Husbands Road and Frontage Road No. 4.

An increase of 1 Working Day was allowed for this Change Order.

CHANGE ORDER NO. 5 (MCCRACKEN COUNTY)

This Change Order was necessary in order to establish bid quantities for excelsior blanket and temporary silt fence. These items were needed to implement the approved "Best Management Practices" or BMP for erosion control on the project.

An increase of 1 Working Day was allowed for this Change Order.

Increase $3,515.00

CHANGE ORDER NO. 6 (MCCRACKEN COUNTY)

This Change Order was necessary to add flexible guide posts type 3 to the contract. While Kentucky Street was closed to traffic, the traveling public was making an

illegal and dangerous turn using private property. Flexible guide posts were installed to discourage this hazardous maneuver.

An increase of 1 Working Day was allowed for this Change Order.

Increase $2,280.00

CHANGE ORDER NO. 7 (MCCRACKEN COUNTY)

This Change Order was necessary to increase steel reinforcement quantities on the project. The plain reinforcement steel for the Husband Road Viaduct over Old Benton Road and the P & L Railroad was underestimated. Also, the spirals for the column reinforcement was changed from #4 bar with a 3" pitch and a 4 ft diameter to a #5 bar with a 3.25" pitch and a 4’-2" diameter. The spacing of the steel reinforcing was not adequate to accommodate the aggregate of the concrete mix. (SEE ATTACHMENT)

An increase of 3 Working Days was allowed for this Change Order.

CHANGE ORDER NO. 8 (MCCRACKEN COUNTY)

This Change Order was necessary to facilitate an approved Value Engineering Change Proposal submitted by the Contractor. The Value Engineering proposal relocated the New Jersey barrier wall to the curb/gutter line and separates the sidewalk from vehicular traffic. This change provided pedestrian safety and resulted in the elimination of the sidewalk concrete and reinforcing as indicated on the contract plans.

Contract Working Days were unaffected by this Change Order.

Decrease $63,204.95

CHANGE ORDER NO. 9 (MCCRACKEN COUNTY)

This Change Order was necessary to add a bid item for Asphalt Pave Milling and Texturing to the contract. This was necessary because the existing grades on Old Benton Road were incompatible with the grades on the new construction. Between the time the plans were prepared and the project was let, Old Benton Road was resurfaced which necessitated a transition to tie in to the plan grades.

An increase of 1 Working Day was allowed for this Change Order.

Increase $7,650.00

CHANGE ORDER NO. 10 (MCCRACKEN COUNTY)

This Change Order was necessary to substitute guardrail end treatment type 4A to guardrail end treatment type 7. The guardrail on the south end of the Husband Road Bridge is located in the utility strip between the sidewalk and the curb. The Type 4A end treatments specified in the contract could not be installed properly and had to be replaced by Type 7 end treatments. Change Order No. 8 moved the barrier wall on the bridge to separate vehicular traffic from the sidewalk. This necessitated placing guardrail in the utility strip, which didn’t allow enough room for the Type 4A End treatment, which requires a flared installation.

Contract Working Days were unaffected by this Change Order.

Decrease $1,650.00

CHANGE ORDER NO. 11 (MCCRACKEN COUNTY)

This Change Order was necessary to establish bid items to remove a tree and reconstruct two manholes. The contractor was required to completely remove a dead tree and stump on Parcel 56. Even though this tree was off right-of-way, the adjacent road construction apparently killed the tree, which created a hazard for the property owner.

During construction, two manholes, which were not shown on the plans, were discovered in the intersection of Wayne Sullivan Drive and Beltline Highway. These manholes required new castings and reconstruction in order to conform to new grades.

An increase of 1 Working Day was allowed for this Change Order.

CHANGE ORDER NO. 1 (LIVINGSTON COUNTY)

This Change Order was necessary to replace the Type 4 Guardrail End Treatments with Type 4A End Treatments in accordance with new specifications.

Contract Working Days were unaffected by this Change Order.

Increase $3,456.47
