Within this site are application checklists to guide you in the development of the Section 5303, 5304, 5307, 5339, 5310, and 5311 applications for Fiscal Year 2014 assistance. Please read these guidelines thoroughly. There is a list of Certifications, Assurances, Model Clauses, and Required Forms for each grant listed below. These checklists and assurances are the only versions of the forms that will be accepted with the application(s). Please read Federal Grant Application Letter then review the checklist and forms carefully as numerous changes, additions, etc. have been made to previous year's grants.
